First Nations Peoples Health essay: Exploring a primary health initiative aimed at reducing the disparity of healthcare for first Nations people. IAHP is Australia’s only Aboriginal and Torres Strait Islander Health Program that overarches all other programs. IHAP had three major goals, first was to improve the health of indigenous/First nations people, especially in remote areas. Secondly, they wanted to increase First Nations people’s access to high-quality and culturally appropriate primary health care services. Lastly, their goal was to improve operations of healthcare services, in a way that was more suitable for the First Nations people (Australian Government Department of Health and Aged Care, 2021). In order to reduce the disparity in healthcare for First Nations people, IHAP have developed strategies to fund healthcare service providers under four themes. First, they focus on providing funds to primary service providers that provide improved immunizations or service delivery to First Nations people. Second, funding is provided to primary service providers that focus on increasing culturally appropriate skills of healthcare professionals. Under third and fourth themes, funding is made available to primary service providers that target health activities like managing chronic conditions and buying or upgrading healthcare infrastructure respectively (AGDHAC, 2021). Australian Government had appropriated $3.292 billion (GST exclusive) for commencing IAHP in 2016. Currently, IAHP has only been able to successfully fund primary service providers that provided healthcare facilities to targeted health issues like eye, ear and oral health, chronic diseases, STDs and substance abuse. IAHP have spent about $161 million on funding the primary service providers that focused on building or upgrading facilities such as accommodation for staff and ACCHSs. IAHP funding improved the condition of healthcare service providers pan Australia. About $62 million has been spent on over 300 clinics to enable them to successfully deliver holistic and culturally appropriate primary health care services. During my second week of final year placement, in paediatric ward, a 14 year old boy having symptoms like severe breathlessness and strain in chest muscles, was admitted. He was accompanied by his mother who reported that the child had consumed peanuts. As it was a busy day in paediatric ward, the RN asked me to examine the patient on my own. After conducting a few preliminary tests on the patient, I found out that his oxygen saturation was 78 and without performing any further tests, I came to a conclusion that the patient might be suffering from anaphylactic shock because he had consumed peanuts.
FEELINGS
The second part of the reflection provides an overview of the emotions and thoughts that the nursing student felt while going through the situation.
As it was my second week of placement, I was very nervous while performing assessments on my own without any guidance. I was also very confused because symptoms that the patient showed were same for both chronic asthma attack and anaphylactic shock.
EVALUATION

The mother of the patient informed me that he had consumed peanuts and his symptoms like severe breathlessness and chest muscle tightening and oxygen saturation which was less than 90, were similar to that of anaphylactic shock (McLendon, 2023). So instead of assessing any further, I decided to treat the patient for anaphylactic shock only. As per ASCIA (2023), I followed the accurate nursing protocol for managing airway and breathing difficulties and the thing that went well in this incidence was that I stayed within the ethical guidelines while treating the patient’s symptoms and that I remained within the scope of my practice. The patient was able to breathe easily and tightening of chest muscle also improved for a while. I think I took this decision because the symptoms like severe breathlessness, tightening of chest muscles and drowsiness is common for both chronic asthma attach and anaphylactic shock and allergic response is very common after consuming peanuts (Greenhawt, 2018).
ANALYSIS

I think the positive aspect of this situation was that I knew how to stay within the ethical guidelines while performing patient assessment. The thing also went well was that I used my previous knowledge about medication and dosage and administered a safe amount of medicine to avoid any adverse effects on the patient, the second time. Another thing that went well was that after getting readmitted, I did not panic which is why I was able to make sensible decision to improve patient’s health (Sockrider & George, 2019). I also applied my skill of revising and creating patient-centred care plan according to the new recent diagnosis of chronic asthmatic attack (Vijay et al., 2019). What didn’t go well was that the patient’s symptom returned and treatment given was not accurate for the condition that he was suffering. I think that lack of guidance, poor decision making and lesser experience was the reason why this situation escalated. I think that RN’s knowledge and experience would have helped me in preventing such a situation. López-Ibort (2021) has investigated that it is one of the major role of a RN to supervise the student nurses at the time of assessments.

This incident help me learn the skill of critical thinking and highlighted the importance of proper assessments. Nurses apply critical thinking in the process of solving problems of their patients and making decisions with creativity to enhance the effect. I still feel like that incident was the biggest learning experience in my nursing journey since it made me realise the importance of critically analysing and assessing the patient to ensure well- being.
